Home Health Aide

Your loved one needs assistance to take care of themselves, but you want them to enjoy the comforts of home.

Who It’s For:

When a loved one needs help with personal care activities such as bathing, dressing or eating, home health aides provide a personalized care solution.

A home health aide can allow your loved one to remain in the comfort of their own home while benefiting from the security and peace of mind that comes with professional care.

What It Is:

Our dedicated, highly-trained aides can assist your loved one with daily tasks that may be difficult for them, such as:

  • Grooming
  • Meal Preparation and Diet Monitoring
  • Light Housekeeping
  • Laundry
  • Walking and getting around the house
  • Medication Reminders
  • Bathing
  • Dressing
  • Personal Care
  • Alzheimer’s and Dementia Care

Our aides are certified by the State of New Jersey and monitored continually by our registered nurses to ensure the highest quality of care for your loved one.

How do I know know if this care is right for me?

If you think your loved one may benefit from the care of a certified home health aide, we will schedule an in-home assessment, where a Towne Representative will meet with your loved one and design a care plan specifically for them.

While all of our aids are highly capable and qualified, we take special care to match clients and aides with compatible personalities. We know that the right caregiver can make all the difference, and we are committed to providing your loved one with a support professional who will care for them as they would for their own family member.

What schedule of care is right for me?

Because we recognize that needs and preferences differ among our clients, Towne home health aides are available on two scheduling options: live-in and hourly.

Our live-in aides stay with their care recipients around the clock, helping with all personal care needs and activities of daily living. They are a constant presence of care and support for seniors who need extensive help and supervision in their day-to-day lives.

Our hourly aides visit and help for a set number of hours, providing support while respecting your loved one’s privacy and independence. Every hourly plan is flexible and built around the client’s needs and preferences.

Skilled Nursing

Staying on top of a loved ones medical needs is stressful. Get the professional care you need, with the warm touch of someone who cares.

Who It’s For:

Your loved one may have healthcare needs that demand expert care and attention. If this is the case, get them the best possible care by enlisting one of our registered nurses.

What It Is:

Towne Nurses are certified by the State of New Jersey and can assist your loved one with:

  • Medication Instruction and Management
  • Medbox Prefills
  • Wound Care
  • Foley Catheter Changes
  • Other Home Healthcare Procedures

If your loved one needs help with activities of daily living in addition to medical care, consider enlisting the services of a home health aide or companion as well as a skilled nurse. By building a custom team of professionals, you ensure that your loved one is provided for in every way possible.

Care Management

Learning to manage the impact of aging or illness on daily life can feel stressful, but sometimes all you need is the right person to guide you and create a plan to ensure specific needs are met.

Who It’s For:

If you have personal care assistance but need additional support in managing and co-ordinating your loved one’s care, a Care Manager may be right for you.

This add-on service relieves the burden of logistical planning, so you spend your time enjoying your loved one, not coordinating their care.

What It Is:

We provide loving and caring care managers who go above and beyond to assist individuals with:

  • Home Setting Evaluations
  • Patient Advocacy
  • Coordinating Medical Appointments
  • Setting up transportation to appointments or outings
  • Meal Delivery
  • Pharmacy support
  • Financial Planning
  • Interviewing and hiring caregivers
  • Obtaining medical supplies and personal care equipment
